SMART relocates to Harlem Neighborhood Health Action Center (2016)

SMART has recently relocated to a new space in East Harlem. The NYC Department of Health and Mental Hygiene, through its Center for Health Equity, has launched Neighborhood Health Action Centers as part of New York City’s plan to promote health equity and reduce health disparities at the neighborhood level. SMART was accepted into this initiative to help with their goals to:
1) Connect neighborhood residents to clinical, community health, and social services through health promotion and referral management to advance comprehensive care coordination and improved neighborhood health outcomes;
2) Reduce service duplication and close service gaps through partner alignment;
3) Establish neighborhood specific action plans to inform collaboration, interventions and investments.

The organization will be co-located with like-minded community organizations at the HARLEM NEIGHBORHOOD HEALTH ACTION CENTER opening soon at 158 East 115th Street New York, NY 10029.

SMART (Sisterhood Mobilized for AIDS/HIV Research and Treatment) is a community-based HIV treatment, health and prevention program that addresses the physical, psychological and social needs of women living with or affected by HIV in a supportive environment. The organization has helped thousands of women since it was founded in 1998 by CHLP Advisory Board Member Susan Rodriguez.
